The planner I create for you will track health issues that you specify, a TV show log, a reading log, a log to keep track of any hobbies you are working on that you specify, your manually paid bills every month, a cleaning schedule, family and friend gift ideas, and a weight-change log. All of these are customizable and you can request to add in or leave out certain logs.
The planner itself is broken up between days and keeps track of work appointments, to-do after work, meals, exercise, mood and health changes, and a notes section.
Only minor details will need to be worked out for what you want to be in your planner.
If you have no way of printing off the pages and turning them into a notebook, I would be willing to do it myself at a larger price.
